Medication Management Services in Greater Philadelphia
and throughout PA.
Hello, I’m Melissa Brubaker, a psychiatric nurse practitioner and founder of Rooted Mind Psychiatric Services.
I provide medication management services for adults residing in Pennsylvania, with convenient telehealth visits and in-person visits coming soon. I strive to provide affirming and respectful care for all, and welcome individuals from all backgrounds and identities.
Just as trees rely on strong roots to stay grounded, mental health depends on a solid foundation, especially when navigating depression, anxiety, or other mood-related concerns. My role is to help strengthen that foundation through thoughtful medication choices, while always treating you as a whole person and not just a diagnosis.
In addition to medication, I offer supportive guidance to help you build routines, set achievable goals, and develop strategies that support mood, energy, and overall well-being. Sessions are collaborative and designed to support sustainable progress in everyday life.
At Rooted Mind, I believe rooted minds don’t just survive, they rise.
VIEW SERVICES
Your initial 60–75 minute evaluation is an opportunity for us to explore your symptoms, history, and goals, understand your needs, and develop a meaningful, personalized
treatment plan together.
* You’ll receive an intake packet to be completed at least 24 hours before your appointment to help gather background information and guide our discussion.
Your 30-minute follow-up visit is an opportunity to track your progress, address any medication concerns, and adjust your treatment as needed to support your mental health goals. If indicated, lab studies and/or Genesight testing may be ordered. I also assist with deprescribing when appropriate, carefully evaluating risks and making gradual, monitored changes.
Supportive guidance is offered during follow-ups from a whole-person perspective. This may include discussion of daily habits such as sleep, nutrition, supplements, and routines,
as well as helping you process concerns, clarify next steps, and explore practical ways to manage daily stress and functioning.
* Please note, this is not a replacement for ongoing comprehensive therapy with a separate therapist.
I accept insurance (see FAQ for accepted plans)
or private self-pay ($225 for initial evaluation, $150 for follow-ups).
Please note: Virtual visits legally require you to be in Pennsylvania at the time of the visit.
Here are answers to some common questions to help you understand what to expect at Rooted Mind and how we work together.
I work with adults ages 18 to 64 across Pennsylvania, providing medication management with practical supportive guidance for daily life. I treat depression, anxiety, trauma/PTSD, OCD, and other mood-related concerns.
This practice does not prescribe controlled substances such as benzodiazepines or hypnotics, except for stimulants when medically indicated.
ADHD diagnosis and treatment typically require an in-person assessment, though an exception may be made if you have a recent neuropsychological evaluation confirming the diagnosis. Prescriptions may not be provided on the same day if additional information, such as medical history or documentation, is needed to ensure safe treatment. Non- stimulant options, such as Strattera or Wellbutrin, may also be considered. Stimulants will not be prescribed if you are taking other controlled substances or using illicit substances.
My care focuses on medication management with supportive guidance. I do not provide standalone therapy. If you’re seeking therapy only, I can refer you to trusted providers.
This practice is not a good fit for individuals needing immediate crisis support, on-call availability, or intensive services. This includes active suicidal or homicidal thoughts with plan and intent, psychosis, severe substance use, or frequent psychiatric hospitalizations. These concerns are best supported in settings with more comprehensive resources, and I can help direct you to appropriate care.
I do not offer after-hours or crisis services. If you are experiencing a mental health crisis or emergency, please seek immediate support using the resources below:
