Hello, I’m Melissa Brubaker, a psychiatric nurse practitioner and founder of Rooted Mind Psychiatric Services.
I provide telehealth psychiatric medication management for adults aged 18-64 in Philadelphia and throughout Pennsylvania. I'm currently self-pay only, with plans to accept insurance in the near future. I strive to offer affirming care for individuals of all backgrounds and identities.
Just as trees rely on strong roots to stay grounded, mental health depends on a solid foundation, especially during periods of overwhelm, sadness, or significant stressors. Whether you're experiencing burnout, anxiety, depression, or other mental health concerns, my role is to help strengthen your roots. That starts with thoughtful medication management, and often includes other tools to support you along the way.
At Rooted Mind, you do not have to navigate mental health challenges alone. I'll be alongside you as we work toward meaningful and sustainable change.
Your initial 60–75 minute psychiatric evaluation lets us explore your concerns, contributing factors, and goals, so I can better understand your needs and develop a personalized treatment plan together with you.
* You’ll receive an intake packet to complete at least 24 hours before your appointment to help gather background information and guide our discussion.
Your 30-minute follow-up visit is an opportunity to monitor your progress, address medication concerns, and adjust treatment as needed to support your mental health goals.
Lab studies, GeneSight testing, or a discussion of supplements and lifestyle factors may also be included, as well as deprescribing, when appropriate, through gradual and carefully monitored changes.
Your follow-up visit may also include brief therapy and support, because medication is only one part of feeling well. We might talk through sleep, routines, or daily stress, process what's on your mind, and work through practical next steps together.

I work with adults ages 18 to 64 across Pennsylvania, providing medication management with practical supportive guidance for daily life. I treat burnout, stress, anxiety, depression, bipolar disorder, panic disorder, trauma/PTSD, ADHD, obsessive-compulsive disorder (OCD), and other mood-related concerns.

Follow-up visits are typically scheduled during your appointment. Established clients can also schedule through the IntakeQ client portal at rootedmind.intakeq.com/portal
Sessions are currently telehealth only. You will receive a secure HIPAA compliant video link (Doxy.me). Please note that both audio and visual are required. You must be located in Pennsylvania during the visit. Please refrain from driving and smoking during visits.
I'm currently self-pay only, with plans to accept insurance in the near future. Initial visits are $225 (60-75 minutes) and follow-up visits are $125 (up to 30 minutes).
This practice does not prescribe benzodiazepines or hypnotics. These medications are intended for short-term support for anxiety and sleep, with long-term use carrying significant risks. Stimulants may be prescribed when medically indicated.
ADHD support and treatment may require a recent neuropsychological evaluation confirming the diagnosis. Stimulant prescriptions are not prescribed at the first visit and require further medical assessment and other documentation to ensure safe treatment. Non-stimulant options, such as Strattera or Wellbutrin, may also be considered. Stimulants will not be prescribed if you are taking other controlled substances or using illicit substances.
While brief therapy and supportive interventions may be included in follow-up visits, my practice primarily focuses on medication management rather than standalone ongoing therapy. If you’re seeking therapy only, I’m happy to refer you to trusted providers.
Follow-up frequency depends on your individual treatment plan, but visits are typically scheduled every 2-4 weeks, especially early in care or after any medication changes. Once you're stable on a medication regimen, follow-up visits can be scheduled up to every 3 months.
Refill requests should be made through the client portal at least one week before you run out. You'll typically receive refills during a visit that covers you until your next scheduled visit. A refill may require a visit to ensure your treatment plan is still appropriate, and medication changes are only made during a visit.
This practice is not a good fit for individuals needing immediate crisis support, on-call availability, or intensive services. This includes active suicidal or homicidal thoughts with plan and intent, psychosis, severe substance use, or frequent psychiatric hospitalizations. These concerns are best supported in settings with more comprehensive resources, and I can help direct you to appropriate care.
Cancellations require 24 hours’ notice. Late cancellations, rescheduling or no-shows of less than 24 hours will incur a $50 fee. If you do not show up to your appointment by 15 minutes from the start time, it will be considered a no-show.
Disability, workers' compensation, forensic, and emotional support animal documentation are not provided. Any other requests for forms or letters should be made ahead of time and will be completed during your appointment. Depending on the request, I'll let you know whether it can be completed.
